Membership Registration
Following is a guideline to help you as process this important paperwork
All forms are processed by our Service Unit and no longer sent directly to the Council office.
There are four (4) ways to submit your forms:
- Drop off at Tina’s house, 11231 Procyon Ct, Rockford MI 49341, 863-4816
- Bring these to each Leader’s Meeting
- Mail them to Tina’s house, 11231 Procyon Ct, Rockford MI 49341 (be sure to use the correct postage!)
- Drop off at school drop boxes at Belmont, Lakes or NRMS
You will be provided with a payment receipt for your records. Receipts will be available in your file at the next leader’s meeting.
Completing the Forms:
- There are three registration forms that are required:
- Girl Registration (green)
- Adult Registration (blue)
- Registration Summary (brown). The summary form must be included every time you turn in forms. This summary is filled out as a check and balance for the girl/adult registrations you are currently submitting. Even if you are only adding one girl or one adult, you need to include another summary form.
- Each form has two copies. Only send in the white copy. Keep the yellow copy for your records.
- It is very important that you fill out the appropriate Council code,
service unit, Association, and troop number on each form. These forms
are used all over the U.S., so you need to include the following information:
- Council Code 450
- Association Area (leave blank)
- Service Unit 322
- Your FIVE-DIGIT Troop number (if one has not been assigned indicate “new”)
- Review each form to ensure that the information is complete and accurate! Pre-printed forms from Council must still be reviewed for accuracy and parent/emergency contact/racial/ethnic information completed.
- If the parent has not filled out the racial information on the child or themselves for adult registrations, you need to do this by asking or visual confirmation. VS1 is the visual survey if you’ve seen the girl yourself. This is a United Way requirement and is a measurement tool commonly used in all industries to determine the effectiveness of providing a well balanced program.
