Because of the Patriot Act, all

38-136-69-24. When opening a new account, your account “name” should
read as follows to be in compliance with IRS rulings:
1st line: Girl Scouts of Michigan Shore to Shore troop#____
2nd Line: c/o troop leader’s name
3rd line troop leader street address
4th line troop leader city state and zip
There must be a minimum of two unrelated volunteer signatures and one SU team member on the signature card. All signers must also sign a Bank Authorization Form.
